GUIDELINES:
- A fund has been established for the purpose of assisting all Sullivan School District full and part-time teachers, custodians, bus drivers, para-professionals, secretaries, cafeteria workers, nurses, and their immediate families to include spouse and/or children in the event of serious illness, house fire, flood, and/or accident.
- The name of the fund is the "SMSTA Good Samaritan Fund."
- Money for the fund will come from SMSTA fundraisers.
- The SMSTA Good Samaritan Fund Committee in charge will consist of two teachers who are SMSTA members from each of the SHS, SMS, SES, and SPS campuses. Teachers will volunteer to serve on this committee.
- Sullivan School District employees may request donations from the SMSTA Good Samaritan Fund by completing and submitting a DONATION REQUEST form secured from an SMSTA building representative, a GSF Committee member, or the Sullivan School District website.
- A DONATION REQUEST form may be requested but not completed by any Sullivan School District employee on behalf of another Sullivan School District employee.
- Donation requests in the amount of $25, $50, $75, or $100 may be made.
- Beginning on January 1, 2012, a staff member may submit two requests per calendar year.
- Requests will be evaluated and approved by the SMSTA Good Samaritan Fund Committee. A quorum of one member from each campus will be necessary to make a decision.
- Requests will be granted based in part on availability of funds.
- Sullivan School District - SMSTA Good Samaritan Fund guidelines will be evaluated and necessary changes will be made as needed by the SMSTA Good Samaritan Fund Committee.
